In order to participate in intercollegiate athletics, all student-athletes are required to complete the SMCCCD Pre-Participation Sports Screening packet and be medically cleared by a physician. Download the packet that corresponds to your status:
NEW Student-Athlete PPE Packet
A "new" student-athlete is any individual who has not previously submitted a completed pre-participation physical packet to Cañada College or the other SMCCD campuses (Skyline College and College of San Mateo). This includes recently graduated high school seniors and individuals who are transferring from a non-SMCCD college/university.
RETURNING Student-Athlete PPE Packet
A "returning" student-athlete is any individual who has previously submitted a completed pre-participation physical packet to Cañada College or one of the other SMCCD campuses (Skyline College or College of San Mateo). If transferring from one of the other campuses, an individual may opt to use this packet but will need to provide a copy of the previously completed NEW student-athlete packet. Alternatively, the individual can opt to complete another NEW student-athlete packet.
To ensure that the packet is completed properly:
- The student-athlete is responsible for completing all pages except for the screening page (the one with a box on the bottom of the page for a physician stamp).
- Download and save the blank PDF forms to your computer before typing content. Typed content may not save if typed before saving.
- Please enter all information, including medical history, truthfully. Any misinformation could result in unexpected consequences.
- Please sign off where indicated. Any student-athlete under 18 will need a parent to sign off as well.
- If any Yeses are checked in the medical history, please explain.
- A MD, DO, NP, or PA must sign off on Page 3. Physicals will NOT be accepted if they are completed and signed off by physical therapists, chiropractors, or medical personnel with another designation.
- Return completed forms to the Athletic Trainers prior to participation.
